    • Serological
    • Is there any evidence of syphilis? Yes _____ No _____ If ‘yes’ please describe condition:

     

    • EKG – Copy of report required
    • Chest X-Ray – Copy of report required
    • Eye Exam by Optometrist or Ophthalmologist – Copy of report required

    • EKG – Copy of report required (EKG required one time until fighter turns 30. 30 to 40 years of age an EKG is required every 2 years. After 40 years old an EKG is required every year.)

    • Eye Exam by Optometrist or Ophthalmologist – Copy of report required (Required once a year. Date of the exam must be within 1 year of the fight date.)
